Lost your degree certificate or need a copy?
The Department of Records and Information Services (DIV) of the UvA can provide a certified copy of your degree certificate and transcript. If you have lost your degree certificate you can request an extract from the examination register. DIV can also provide a statement of graduation or a statement of registration.

Please note that the DIV will process and assess your document request. Explain your request as fully as possible. Forms are usually processed within two weeks. There is no charge for this service.
Need a copy of degree certificate
If you have your original degree certificate and transcript you can request a certified copy of these documents. Fill out the form below and attach a scan of your degree certificate or transcript and of a valid ID card.
It is also possible to request a certified copy at the Student Service Desk. You can have your certified copies made at the Student Service Desk upon presentation of the original documents and a valid ID card.
Lost degree certificate
If you have lost your degree certificate an extract from the examination register can be provided. Apart from the original degree certificate (which is issued only once), such a certified copy is the only official proof of graduation.
If you lost your transcript and graduated after 1995, we can provide a certified list of your grades. Unfortunately, these list are often incomplete.

Need a statement of graduation or a statement of registration
In some situations, an (additional) statement can be required:
- a (copy of the) certificate or extract from the examination register is not available;
- a foreign language document is needed;
- a copy of your diploma or a copy of the exam register is for other reasons not sufficient, for example when applying for a visa.
If you need a statement of registration, for example in the context of a pension application, you can also use this form.
